Business Forms - New Programs

Community Child Care Connection, Inc (CCCC) is part of a statewide, nonprofit resource and referral system funded through the Illinois Department of Human Resources. We develop and maintain a database of child care providers, including both centers and homes. We assist parents in locating child care and provider information on what is quality care.
 
To become an active provider in our database, one must complete and return the following CCCC forms:

• Introduction letter
• Checklist
• Application with Self-certification
• CANTS Background Check

A family child care license exempt provider must also turn in a Health Report Form.

Once a provider is listed in our database, they are eligible for the following services through our offices:

• Program Referrals to Parents
• In House Trainings
• Professional Development Funds
• Lending Library Access
• Opportunity to apply for Quality Counts Grants
